Home and Community-Based Elderly and Disabled Waiver Program
Through participation with the Mississippi Division of Medicaid, the Central Mississippi Planning and Development District (CMPDD) provides access to home and community-based services for individuals who are at risk of institutionalization and require the level of care typically provided in a nursing facility.
CMPDD’s Case Management staff—comprised of Registered Nurses and Licensed Social Workers—coordinate and arrange for the delivery of all Elderly and Disabled (E&D) Waiver services in partnership with Medicaid-approved providers. All services must be pre-approved by the Division of Medicaid, and costs are reimbursable through Medicaid.
Eligibility Requirements:
To qualify for the E&D Waiver Program, individuals must meet the following criteria:
- Be 21 years of age or older;
- Meet Medicaid eligibility requirements by receiving SSI benefits, or have a monthly income not exceeding 300% of the SSI income limit;
- Score 50 or higher on the Long-Term Services and Supports (LTSS) InterRAI assessment tool; and
- Meet nursing facility level-of-care criteria, requiring assistance with Activities of Daily Living (ADLs) or Instrumental Activities of Daily Living (IADLs).

- In-Home and Institutional Respite Services
- Adult Day Care Services
- Home-Delivered Meals
- Extended Home Health Services
- Assistance During Transportation for Medical Care
- Community Transition Services (for individuals moving from a nursing facility to a community setting)
- Environmental Safety Services
- Medication Management Services
This program is available throughout CMPDD’s service area, which includes Hinds, Madison, Rankin, Warren, Yazoo, Copiah, and Simpson Counties.
